Adoption Application

Please Read Before Applying

This questionnaire was designed to help us evaluate potential adopters.

Pet adoption can place a physical, mental, and financial burden on a household, and K-9 New Life wants to ensure that the adopting family feels they are able to handle the responsibilities that come with adopting a dog.

Please remember that while you may know yourself to be a wonderful dog owner, we do not know you personally and must base our decisions on our prior experiences, operation guidelines, and what we know we want for the dogs in our care.

Completing this application does not guarantee an adoption.
